Patient-Driven Grouping Model (PDGM) is an updated payment model from the Centers for Medicare and Medicaid Services or (CMS) that will begin January 1, 2020. You may remember hearing about the former CMS payment model, called PPS or Prospective Payment System. This module will assist the Clinical team in understanding their role in ensuring our success in PDGM.
