MedStar hospitals will transition to vancomycin area-under-the-curve (AUC) dosing by Pharmacists using the integrated InsightRX software. Compared to trough-based vancomycin dosing (targeting troughs of 10-20 mcg/mL), AUC dosing decreases overall vancomycin exposure and nephrotoxicity while improving patient outcomes. AUC dosing is advocated in the 2020 American Society of Health-Systems Pharmacists, Infectious Diseases Society of America, Pediatric Infectious Disease Society, and the Society of Infectious Diseases Pharmacists joint consensus guidelines for vancomycin monitoring for MRSA infections, and will be used as the standardized approach at MedStar hospitals for managing IV vancomycin in adults (except in patients on renal replacement therapies, those with acute renal impairment, or for surgical prophylaxis).
