This learning module will follow the evolution of home health nursing from the early 19th century to the collaborative, efficient, and effective caregiving services of today. This learning module will define the expansion of skilled services and will review the progression of reimbursement to demonstrate the financial challenges faced by patients. This learning module will also cover mandates implemented to cover services for patients who require intermittent skilled services but who are confined to the home, as leaving would be contraindicated. You will see the value of the partnerships and integrations of home health skilled professionals outside the walls of the home that help create a patient-centered care plan with measurable process and outcome indicators and specific accreditation criteria. You will understand how patients’ quality of care is measured and how performance improvement systematically improves the individual health experience, as well as the reduction of cost that extends beyond home health to the continuum of care.
