Congestive Heart Failure (HF) is a chronic progressive disease and a growing public health concern due to its long-term implications, disease management, continued treatment regimen, and high mortality rate. HF is a leading cause for mortality and morbidity, with a prevalence of 6 million in the United States and a healthcare cost of more than $30 billion per year for emergency room visits and readmissions (Centers for Disease Control and Prevention [CDC], 2020). Care coordination, post-hospitalization follow-up, home care management, and patient/family engagements are crucial in managing HF patients at home.
