The SBIRT process in an inpatient setting is a structured, evidence-based approach nurses use to identify and address risky substance use among hospitalized patients. The SBIRT inpatient nurse screening process begins with the nurse administering a validated tool (such as the AUDIT-C or CRAFFT 2.0) during admission to identify risky or problematic substance use. If a patient screens positive, the peer recovery coach is notified in the EMR, who then delivers the brief intervention—a short, motivational conversation aimed at raising awareness, encouraging reflection, and supporting readiness for change. For patients with high-risk or severe use, the peer recovery coach collaborates with the clinical care team to provide a warm handoff to specialized treatment behavioral health services. All screenings, interventions, and referrals are documented in the electronic health record, ensuring continuity of care and follow-up during the hospital stay.
