- Explain why patient safety event reviews are conducted.
- Discuss how safety event reviews are related to Just Culture and High Reliability Organization (HRO) principles.
- Apply safety and human error models to the safety event review process.
- Demonstrate effective data collection and interviewing techniques.
- Apply approaches to create an event timeline and determine contributory factors.
- Generate effective event review reports that include system-focused contributing factors, high-risk unrelated factors, and recommendations for action plans.
