Schedule an appointment for your flu shot at the District of Columbia Home Care Agency. 

Location: 4201 Connecticut Ave NW Washington, DC 20008 Suite 200

When you report for your appointment:

• Wear required PPE
• Bring 2 barriers AND pens to fill out vaccine forms
• Follow the COVID protocols for the agency
