Objectives
- Access and complete the RRT PowerForm within the EHR through Ad-Hoc documentation.
- Accurately document the qSOFA score and RN name during every Rapid Response event.
- Calculate and document the Glasgow Coma Scale (GCS) as part of qSOFA.
- Communicate and escalate a positive qSOFA score (≥ 2) in accordance with clinical policy.
- Explain the purpose of the qSOFA tool and its role in early sepsis recognition.
- Identify and score the three components of qSOFA: systolic blood pressure, respiratory rate, and altered mentation using GCS.
