- This education module provides training for nursing staff on accurate documentation of qSOFA (Quick Sequential Organ Failure Assessment) during Rapid Response Team (RRT) activations. The module will review the purpose of qSOFA as a bedside sepsis screening tool and outline documentation expectations within the electronic health record (EHR).
- Learners will receive step-by-step instruction on how to access and complete the qSOFA through Ad-Hoc, including documentation of vital components such as respiratory rate, systolic blood pressure, Glasgow Coma Scale (GCS), and nurse identification. The module emphasizes the importance of clinical communication and provider notification when a qSOFA score is ≥ 2 (positive).
Objectives
- Access and complete the RRT PowerForm within the EHR through Ad-Hoc documentation.
- Accurately document the qSOFA score and RN name during every Rapid Response event.
- Calculate and document the Glasgow Coma Scale (GCS) as part of qSOFA.
- Communicate and escalate a positive qSOFA score (≥ 2) in accordance with clinical policy.
- Explain the purpose of the qSOFA tool and its role in early sepsis recognition.
- Identify and score the three components of qSOFA: systolic blood pressure, respiratory rate, and altered mentation using GCS.
