Palliative Care & Rehabilitation – Building Bridges Description Speakers will be discussing the philosophy of palliative and hospice care services. There will be a review of a typical palliative care consult session. The speakers will discuss the collaboration between rehab and palliative care services which will include tools to help with collaboration, Prognosis- Role of palliative rehabilitation, Case studies, Review of language and communication, and Discharge planning.
