This course provides an overview of Case Management (CM), including the role this team plays for MedStar Family Choice (MFC), an overview of community resources including their importance, specific programs for Maryland and DC, and how members can be referred to these programs, and an overview of the Biopsychosocial Model and Social Determinants of Health, how to identify how MedStar Family Choice can leverage both in care management to benefit member health status and outcomes, and will also review the Biopsychosocial Model, Social Determinants of Health, and the application to case management.  It also provides a review of the Case Management regulatory requirements and expected standards for productivity, including turnaround times for Maryland and DC.
