De-accessing an implanted venous port is a sterile procedure performed to safely remove a non-coring (Huber) needle from an accessed port following completion of therapy. This process requires adherence to strict aseptic technique to prevent infection, proper flushing and locking of the device per protocol, and application of an appropriate dressing to maintain site integrity. The nurse must ensure patient safety and comfort, confirm completion of ordered therapies, and monitor for complications such as bleeding, occlusion, or signs of infection. show less
Objectives
- Apply appropriate dressing and ensure hemostasis at the site
- Assess the site post-procedure for bleeding, swelling, pain, or signs of infection
- Demonstrate proper aseptic technique throughout the de-accessing procedure to minimize infection risk
- Describe indications for de-accessing an implanted port, including completion of therapy and provider orders
- Educate the patient on post-procedure care and when to report concerns
- Identify required supplies and prepare equipment appropriately prior to the procedure
- Perform correct flushing and locking technique per institutional protocol (e.g., saline and/or heparin lock)
- Recognize and respond to complications, such as resistance during flushing, bleeding, or suspected catheter-related infection
- Safely remove the non-coring needle while maintaining stabilization of the port
