About this Course: Ongoing daily audits conducted by the Stroke Coordinator and Stroke Navigator have identified inconsistencies in the documentation of neurological assessments, including missing assessment components and discrepancies between nursing neurological assessments and the National Institutes of Health Stroke Scale (NIHSS) scores. These findings indicate gaps in assessment completeness, documentation practices, and alignment with standardized stroke assessment protocols.Accurate neurological assessments and consistent NIHSS scoring are critical for appropriate stroke evaluation, treatment decisions, communication among the interdisciplinary care team, and adherence to stroke care standards. Variability in assessment documentation may lead to misinterpretation of patient status, delays in recognizing neurological changes, and inconsistencies in patient care.This learning module was developed to address these practice gaps by reinforcing the components of a comprehensive neurological assessment, reviewing proper NIHSS scoring techniques, and improving the alignment between clinical assessments and standardized stroke scales. Through neuro-focused education, the module aims to enhance assessment accuracy, improve documentation completeness, and promote consistency between neuro assessment and NIHSS to support high-quality stroke care and improved patient outcomes. show less
