Research shows that incorporating person-centered care (PCC) into care delivery improves the patient and family experience and supports better outcomes. This training introduces core principles of person-centered care in the home care setting and prepares participants to serve as mentors who promote these practices within their teams. Learners will explore strategies to support meaningful patient engagement, shared decision-making, and compassionate communication while helping foster a culture that prioritizes patient preferences, dignity, and individualized care.
